慢性阻塞性肺疾病患者吸入药物用药依从性现状调查和影响因素分析

目的 调查慢性阻塞性肺疾病(COPD)患者的吸入药物用药依从性现状,并分析其影响因素。方法 收集2021年2月至2022年2月就诊于COPD专病门诊的COPD患者200例,分别采用药依从性报告量表(MARS)、药物治疗信念问卷(BMQ)和COPD评估测试(CAT)问卷调查患者的吸入药物用药依从性、药物治疗信念和COPD对其生活质量的影响,并采用Logistic回归进行影响因素分析。结果 200例COPD患者中有79.5%的COPD患者吸入药物用药依从性高,根据患者用药依从性分为高依从性组(n=159)和低依从性组(n=41),高依从性组BMQ的必要性得分高于低依从性组(P<0.01),顾虑程度得分低于低依从性组(P<0.01)。BMQ的必要性得分、顾虑程度得分和CAT问卷总分是COPD患者吸入药物用药依从性的影响因素(P<0.05)。结论 临床药师应评估患者的用药信念,关注患者的用药依从性,通过增强COPD患者的药物治疗信念,减少用药顾虑,提高吸入药物用药依从性。

Abstract:

AIM The study extends previous research by examining the current status of adherence to inhaledmedication and analyzing its influencing factors among adult Chinese patients with chronic obstructive pulmonarydisease(COPD). METHOD The survey enrolled 200 COPD outpatients from the COPD specialty clinic betweenFebruary 2021 and February 2022. Data on adherence to inhaled medications, beliefs about pharmacotherapy, and theimpact of COPD on quality of life were assessed using the medication adherence report scale(MARS), beliefs aboutmedicines questionnaire(BMQ), and COPD assessment test(CAT), respectively. Face-to-face interviews were conductedto collect these data. Baseline patient characteristics were analyzed with chi-square tests, and multivariable logisticregression was employed to identify influencing factors. RESULTS Among 200 COPD patients, 79.5% exhibited highadherence to inhaled medication regimens. Based on adherence levels, they were categorized into high-adherence(n=159)and low-adherence(n=41) groups. The necessity score of the BMQ was significantly higher in the high-adherence groupcompared to the low-adherence group(P < 0.01). The concern score was significantly lower in the high-adherence groupcompared to the low-adherence group(P < 0.01). Multivariable analysis revealed that BMQ necessity scores, BMQconcern scores, and CAT total scores were independent influencing factors of adherence to inhaled medications(P <0.05). CONCLUSION Clinical pharmacist should evaluate the patients' drug beliefs, pay attention to their medicationcompliance, improved medication adherence in COPD patients can be achieved by enhancing the medication beliefs andreducing anxiety.
