目的探讨临床药师对脑梗死住院患者实施药学监护的模式和意义。方法结合患者的病理生理情况,对患者住院期间用药进行分析,从药物的选择、给药剂量、给药频率、使用疗程以及药物相互作用方面评估给药方案,进行药学干预。结果患者经治疗后好转出院。结论临床药师利用自身专业的优势,与医师进行知识互补,协助医师作出更准确的给药决策,促进临床合理用药,提高医疗水平。
