目的 探讨度洛西汀治疗紫杉烷类药物引起周围神经病变（TIPN）的临床疗效、不良反应及对生活质量的影响。方法 将2019年2月至2022年8月收治的80例TIPN患者随机分为2组，每组40例。对照组给予常规护理、口服复合维生素（善存）和塞来昔布，观察组在对照组的基础上给予度洛西汀。6周后使用癌症治疗相关神经毒性功能评估表（FACT/GOG-Ntx）评估TIPN的严重程度，使用美国国立癌症研究所通用毒性标准4.0版本（NCI-CTC v4.0）评估不良反应分级，使用癌症患者生命质量测定量表（EORTC QLQ-C30）评估生活质量。结果 在TIPN严重程度方面，对照组和观察组FACT/GOG-Ntx评分分别为11(8,15)vs 8(6,11),2组比较具有统计学差异（P <0.05）；不良反应方面，不良反应分级均为Ⅰ～Ⅱ级，2组差异无统计学意义（P> 0.05）；生活质量方面，观察组EORTC QLQ-C30评分高于对照组，差异具有统计学意义（P <0.05）。结论 度洛西汀治疗TIPN有一定效果，不良反应均为Ⅰ～Ⅱ级，安全性良好，且能改善患者生活质量。
Abstract：AIM To explore the therapeutic effect, adverse reactions and quality of life of duloxetine in the treatment of taxane induced peripheral neuropathy(TIPN). METHODS A total of 80 patients with TIPN admitted from February 2019 to August 2022 were randomized into 2 groups. The control group was given routine nursing and oral multivitamin(shancun) and celecoxib, while the observation group was given duloxetine on the basis of the control group. Follow-up after 6 weeks included the use of functional assessment of cancer therapy/gynaecologic oncology group-neurotoxicity scale(FACT/GOG-Ntx) to assess the severity of TIPN, the use of national cancer institute common toxicity criteria(NCI-CTC) adverse reaction grading, and the use of EORTC QLQ-C30 to assess the quality of life score.RESULTS In terms of the severity of TIPN, the FACT/GOG-Ntx scores for the control group and observation group were 11(8, 15) vs 8(6, 11). There was a statistical difference between 2 groups(P<0.05). In terms of adverse reactions,most of them were graded as 1-2, and there was no significant difference between the two groups(P>0.05). In terms of quality of life, the overall quality of life score of patients in the observation group was higher than that in the control group, with statistical difference(P<0.05). CONCLUSION Duloxetine has a certain effect in treating TIPN. Adverse reactions are mostly graded as Ⅰ ～ Ⅱ, well tolerated, and contribute to an overall improvement in the quality of life for patients.
