乳腺癌是死亡率较高的恶性肿瘤之一,其中激素受体阳性和人表皮生长因子受体2阴性亚型较为常见。内分泌治疗在该亚型乳腺癌治疗中占据重要地位。大量研究证实,联合细胞周期蛋白依赖性激酶4和6抑制剂能明显提升疗效。相较于国内已上市的其他细胞周期蛋白依赖性激酶4和6抑制剂,瑞波西利在临床应用中具有独特优势,其在激素受体阳性和人表皮生长因子受体2阴性晚期乳腺癌的一线治疗中展现出显著的总生存期获益,不仅适用于内脏危象患者,还可特异性抑制代偿性细胞周期蛋白依赖性激酶6-cyclin D3二聚体,防止肿瘤细胞通过旁路激活产生耐药,实现长久的临床获益。现通过综述国内已上市的细胞周期蛋白依赖性激酶4和6抑制剂药物治疗的研究进展,探讨瑞波西利在激素受体阳性和人表皮生长因子受体2阴性乳腺癌中的临床应用,以期为优化我国乳腺癌患者的治疗方案和提升疗效提供参考依据。
Abstract:Breast cancer is one of the malignant tumors with relatively high mortality rates, with the hormone receptor(HR) positive and human epidermal growth factor receptor 2(HER2) negative subtype being prevalent. Endocrine therapy plays an important role in the treatment system of HR+/HER2-breast cancer. Many studies have shown that the combination with cyclin-dependent kinases 4/6(CDK4/6) inhibitors can significantly improve efficacy. Compared with other CDK4/6 inhibitors marketed in China, ribociclib has unique clinical manifestations. In the first-line treatment of HR+/HER2-advanced breast cancer, it demonstrates a significant benefit in prolonging overall survival(OS). It not only applies to patients with visceral crisis, but also effectively inhibits the compensatory CDK4/6-cyclin D3 dimer, preventing tumor cells from developing drug resistance through bypass activation and achieving long-term clinical benefits. By reviewing the research progress of CDK4/6 inhibitor therapy marketed in China, and conducting an in-depth discussion on the clinical application progress of ribociclib in HR+/HER2-breast cancer, we aimed to provide a reference for optimizing treatment regimens and improving therapeutic effects for breast cancer patients in China.
