2型糖尿病患者瑞格列奈不良反应发生情况及其影响因素

目的 探讨2型糖尿病(T2DM)患者瑞格列奈不良反应发生情况及其影响因素。方法 选取2018年3月至2020年12月4 152例接受瑞格列奈治疗的T2DM患者进行回顾性研究,统计瑞格列奈不良反应发生情况。根据不良反应发生与否分为发生组和未发生组,采用SPSS 24.0软件进行单因素、多因素logistic回归分析。结果 接受瑞格列奈治疗的T2DM患者4 152例中,共发生不良反应44例(发生率1.06%),以低血糖(56.82%)、肝损害(11.36%)、便秘(6.82%)和皮疹/瘙痒(6.82%)为多见。单因素和多因素logistic分析均显示,患者年龄、饮酒史、瑞格列奈单次剂量、抗Hp三联治疗、过敏性鼻炎、神经衰弱、营养不良和既往磺脲类降糖药物应用史均是瑞格列奈不良反应发生的影响因素(P <0.05)。结论 T2DM患者瑞格列奈不良反应少见,发生情况可能与年龄、饮酒史和瑞格列奈单次剂量等有关,临床可结合以上信息为患者提供精准的药学服务,以提高用药安全性。

Abstract:

AIM To investigate the adverse reactions of repaglinide in patients with type 2 diabetes mellitus(T2DM) and its influencing factors. METHODS Totally 4 152 patients with T2DM treated with repaglinide from March 2018 to December 2020 were selected for this retrospective study, and the adverse reactions of repaglinide were counted.According to the occurrence of adverse reactions, they were divided into occurrence group and non occurrence group.SPSS 24.0 software was used for univariate and multivariate logistic regression analysis. RESULTS Among 4 152patients with T2DM treated with repaglinide, 44 patients(1.06%) had adverse reactions, most of which were hypoglycemia(56.82%), liver damage(11.36%), constipation(6.82%) and rash/itching(6.82%). The logistic regression analysis showed that age, drinking history, repaglinide dose, anti Hp triple therapy, allergic rhinitis, neurasthenia, malnutrition and previous application history of sulfonylurea hypoglycemic drugs were factors influencing the adverse reactions of repaglinide(P < 0.05). CONCLUSION Adverse reactions of repaglinide in T2DM patients are rare. The occurrence may be related to age, drinking history, single dose of repaglinide and so on. We can provide accurate pharmaceutical care for patients in combination with the above information to improve drug safety in clinic.
