目的 了解国家基本药物在我院胃肠外科使用现状，探讨临床药师通过开展药学服务推动胃肠外科国家基本药物使用的方法和成效，为促进同类医院基本药物的合理使用提供参考。方法 利用HIS系统和医院运营分析平台，调取2020年1-8月（开展药学服务前）和2021年1-8月（开展药学服务后）我院胃肠外科全部住院和门诊患者用药情况，分析临床药师在开展药学服务前后对胃肠外科门诊患者基本药物处方比、住院患者基本药物使用率及药费金额等指标的变化情况，并评价临床药师开展药学服务提升医院基本药物使用率的效果。结果 临床药师开展药学服务后我院胃肠外科门诊患者基本药物处方占比达到50.10%，较药学服务前的平均值43.38%有所提高（P <0.001）。住院患者基本药物使用率维持在99%左右。国家基本药物使用金额占比由20.37%提升至26.79%。结论 通过临床药师开展药学服务，可有效提高我院胃肠外科国家基本药物使用率和基本药物使用金额占比。
Abstract：AIM To understand the current status of the utilization of national essential drugs in our hospital's gastrointestinal surgery department, explore the methods and effects of promoting the use of national essential drugs in gastrointestinal surgery by clinical pharmacists through pharmaceutical care, and provide reference for promoting the rational use of essential drugs in similar hospitals. METHODS Using HIS system and hospital operation analysis platform,the drug usage of all inpatients and outpatients in gastrointestinal surgery department from January 2020 to August 2020(before providing pharmaceutical care) and from January 2021 to August 2021(after providing pharmaceutical care) were collected. The changes in indicators such as the prescription ratio of essential drugs, the utilization of essential drugs and the proportion of the amount of national essential drugs before and after the implementation of pharmaceutical care were analyzed, and the effect of clinical pharmacists providing pharmaceutical care to improve the usage of essential drugs in the hospital was evaluated. RESULTS After providing pharmaceutical care by clinical pharmacists, the proportion of essential drug prescriptions for outpatients of gastrointestinal surgery department in our hospital reached 50.10%, which was higher than the average value of 43.38% before pharmaceutical care(P < 0.001). The usage rate of essential drugs in hospitalized patients remained at around 99%. The proportion of the amount of national essential drugs increased from 20.37% to 26.79%. CONCLUSION The pharmaceutical care provided by clinical pharmacists can effectively increase the usage rate of national essential drugs and the proportion of the amount of national essential drugs in gastrointestinal surgery department of our hospital.
