基于贝叶斯决策工具探讨华法林在成人中的个体化给药效果

摘要:

目的比较基于贝叶斯决策工具的华法林剂量计算软件1.0.1的个体化给药效果与传统华法林给药方法的效果,为临床优化华法林使用提供参考。方法纳入2016年1月-2018年12月呼吸科及心内科住院并接受华法林抗凝治疗的患者162例,随机分为观察组和对照组。观察组采用华法林剂量计算软件1.0.1估算剂量,对照组采用常规给药方法,比较2组临床结果的差异,并分析年龄对给药效果的影响。结果与对照组相比,观察组可以在较短时间内获得较好的目标INR,显著缩短了患者住院时间。年龄对给药效果的影响显示,观察组软件用于年龄≥65岁的老年患者时目标INR预测效果较好。结论基于贝叶斯决策工具的华法林剂量计算软件1.0.1相较于传统华法林给药方法,可以在较短时间内获得较好的目标INR值,具有较好的预测能力,值得在临床上进一步推广和应用。

Abstract:

AIM To provide a reference for the clinical optimization of warfarin administration by comparing the warfarin dose calculation software 1.0.1 based on Bayesian decision-making tool as compared with the traditio-nal warfarin administration method. METHODS Patients admitted to respiratory and cardiology department in our hospital from January 2016 to December 2018 were enrolled, and divided into observation group and control group. In the observation group, the dose was estimated by warfarin calculation software 1.0.1, while the control group was gi-ven a conventional dose. The clinical effects between 2 groups were compared, and the influence of age on the effect of drug delivery was analyzed. RESULTS The observation group generally achieved the target INR in a short period of time, which could significantly shorten the hospitalization time of patients as compared to control group. The analysis of influence of age on the effect of drug delivery showed that the observation group had a good predictive ability for elderly patients over 65 years old. CONCLUSION The warfarin dose calculation software 1.0.1 based on Bayesian decision-making tool has better predictive ability,and can achieve the target INR value in a shorter time than the traditional warfarin administration method, and it is worth further clinical promotion and application.
