目的 探讨人免疫球蛋白（IVIG）治疗重症及危重症病毒性肺炎患者的疗效及安全性。方法 收集2022年12月15日至2023年3月1日收住入院的重型及危重型病毒性肺炎病例210例，其中101例使用IVIG治疗（治疗组），109例未使用IVIG治疗（对照组）。比较2组患者的基线特征、实验室指标、联合用药情况和临床治疗效果。结果 治疗组发热人数、白细胞计数和中性粒细胞计数均高于对照组（P <0.05），淋巴细胞计数低于对照组（P <0.05）;2组联合用药情况比较差异无统计学意义（P> 0.05）；治疗组吸氧时间、住院时间及30 d死亡率均少于对照组，差异均有统计学意义（P <0.05）。结论 IVIG用于治疗重症及危重症病毒性肺炎可以提高治疗效果，缩短治疗时间，降低死亡率。
Abstract：AIM To explore the efficacy and safety of intravenous immunoglobulin(IVIG) treatment in patients with viral pneumonia. METHODS A total of 210 patients of severe and critical viral pneumonia were admitted to the hospital from December 15, 2022 to March 1, 2023. Among them, 101 patients were treated with IVIG and were categorized into the treatment group, and 109 patients were not treated with IVIG and were categorized into the control group. Comparison was made between the 2 groups regarding baseline characteristics, laboratory test results, concomitant medication status, and clinical treatment outcomes. RESULTS In the treatment group, the number of patients with fever,white blood cell count, and neutrophil count were all higher than in the control group(P < 0.05), and the lymphocyte count was lower than in the control group(P < 0.05). There was no statistically significant difference in concomitant medication use between the 2 groups(P > 0.05). The treatment group had a shorter duration of oxygen therapy, shorter hospital stay,and lower 30 d mortality rate than the control group, and these differences were all statistically significant(P < 0.05).CONCLUSION The use of IVIG in the treatment of severe and critical cases of viral pneumonia can enhance treatment effectiveness, reduce treatment duration, and decrease mortality rates.
