报道1例临床药师参与的产超广谱β-内酰胺酶肺炎克雷伯菌尿路感染患者的药学实践。患者，男性，80岁，入院诊断为复杂性尿路感染。入院后首先给予头孢哌酮舒巴坦治疗，患者尿路感染好转，但血红蛋白水平呈渐进性下降，判定很可能为头孢哌酮舒巴坦所致药物不良反应。临床药师经过文献分析发现，头孢西丁可用于产超广谱β-内酰胺酶肠杆菌感染的治疗，并被认为是碳青霉烯类药物治疗ESBL-E所致尿路感染的潜在替代药物，因此建议抗感染方案调整为头孢西丁（2 g,iv gtt,tid，每次静脉滴注持续2 h）。最终患者尿路感染得到治愈，同时血红蛋白水平恢复正常。
