目的探究苁蓉益肾颗粒联合厄贝沙坦治疗早期糖尿病肾病的疗效及对糖脂代谢、氧化应激指标的影响。方法选取2016年1月至2018年1月就诊的早期糖尿病肾病患者94例,采用随机数表法分为对照组(47例)及观察组(47例)。2组均予厄贝沙坦0.15 g,qd,po。观察组在此基础上予苁蓉益肾颗粒2 g,bid,po,疗程10周。对比2组治疗前后尿蛋白、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、血清超氧化物岐化酶(SOD)、丙二醛(MDA)、晚期蛋白氧化产物(AOPP)水平,治疗总有效率及不良反应发生率。结果治疗后,2组尿蛋白、FBG、HbA1c水平均显著下降(P<0.05),其中观察组上述指标显著低于对照组(P<0.05);2组TC、TG、LDL水平均显著下降、HDL值均显著上升(P<0.05),其中观察组TC、TG、LDL水平显著小于对照组(P均<0.01),HDL值显著高于对照组(P均<0.01);2组SOD、MDA、AOPPs水平均显著下降(P<0.05),其中观察组SOD、MDA、AOPPs水平显著小于对照组(P均<0.01);观察组治疗总有效率显著高于对照组(87.23%vs 65.96%,P<0.05)。结论苁蓉益肾颗粒联合厄贝沙坦治疗早期糖尿病肾病疗效显著,可有效改善血糖、血脂、尿蛋白及氧化应激水平。
Abstract:AIM To explore effects of Congrong Yishen granules combined with irbesartan in the treatment of early diabetic nephropathy and on glucose and lipid metabolism and oxidative stress. METHODS From January 2016 to January 2018, 94 early diabetic nephropathy patients diagnosed and treated in our hospital were selected, and the random number table method was used to divide patients into control group(47 cases) and observation group(47 cases) respectively. Both groups were given irbesartan, 0.15 g, qd, the observation group was given Congrong Yishen granules on this basis, 2 g, bid. The total therapeutic efficiency, urine protein, adverse effects rate and fasting blood glucose(FBG), hemoglobin A1 c(HbA1 c), triglyceride, cholesterol(TG), high-density lipoprotein(HDL), low-density lipoprotein(LDL), serum superoxide dismutase(SOD), malondialdehyde(MDA), advanced protein oxidation products(AOPP) before and after treatment between 2 groups were compared. RESULTS There was no significant difference in urine protein, blood glucose, blood lipid and oxidative stress between 2 groups before treatment(P>0.05). After treatment, urine protein, FBG and HbA1 c decreased significantly in both groups(P<0.05). The above indicators in observation group were significantly lower than those in the control group(P<0.05). After treatment, TC, TG and LDL were significantly decreased and HDL was significantly increased in both groups(P<0.05). TC, TG and LDL of observation group were significantly lower than those of control group(all P<0.01), and HDL of observation group was significantly higher that of than control group(P<0.01). After treatment, SOD, MDA and AOPPs decreased significantly in both groups(P<0.05), the above indexes in the observation group were significantly lower than control group(all P<0.01).The total effective rate of the observation group was significantly higher than that of the control group(87.23% vs 65.96%,P<0.05). CONCLUSION The treatment of early diabetic nephropathy by Congrong Yishen granules combined with irbesartan is effective, and it can effectively improve the glucose, lipid urine protein and oxidative stress.
