报道1例老年急性淋巴细胞白血病患者化疗后继发中性粒细胞缺乏，并出现耐碳青霉烯类铜绿假单胞菌（CRPA）与黑曲霉菌肺部感染的抗感染治疗实践过程。患者，男性，65岁，临床诊断为急性淋巴细胞白血病，入院拟行化疗。入院后患者反复发热，临床药师结合检查结果，为患者制订了个体化抗感染治疗方案：针对CRPA感染，采用以多黏菌素B为基础的联合治疗方案；针对黑曲霉菌肺部感染，采用艾沙康唑联合卡泊芬净的治疗方案。通过精准的抗感染策略与及时调整用药方案，该患者2种耐药病原体感染均得到了有效遏制，临床症状与影像学表现均获显著改善。
