目的探讨应用PDCA循环法规范医院甲状腺手术、乳腺手术、腹股沟疝手术3种Ⅰ类切口手术围术期预防用抗菌药物合理使用的效果。方法运用PDCA循环方法收集资料,找出存在问题,分析原因,制定整改措施,督促落实措施,检查和持续改进。对实施PDCA循环前的2010年5—7月及实施PDCA循环后的2011年4—6月3种Ⅰ类切口手术出院病历预防用抗菌药物的合理性进行对照研究。结果应用PDCA循环法后,抗菌药物使用率显著下降,由实施前的100%下降为实施后46.67%(P<0.01);不合理使用抗菌药物的现象明显得到改善。结论应用PDCA循环法可有效规范I类切口手术抗菌药物的合理使用。

AIM To investigate the rational use of antibacterial drugs at perioperative period of three types I incision operations including thyroid surgery,breast surgery,inguinal hernia surgery and survey the effect of the plan,do, check and action(PDCA) cycle management.METHODS According to PDCA procedure,materials were collected and analyzed,and measures were made out on promotion followed by supervising the practice for further improvement.The data of medication of two groups from May to July 2010(before the implementation of PDCA method) and from April to June 2011(after the implementation of PDCA method) were collected and compared.RESULTS After implementation of PDCA method,the percentage of antibacterial drug use significantly decreased,from 100%to 46.67%(P < 0.01). The irrational uses of antibacterial drugs were significandy improved.CONCLUSION PDCA cycle management for standardizing the rational use of antibacterial drugs are effective and feasible.The method can contribute to improving the rational use of antibacterial drugs at perioperative period.
