目的观察丁丙诺啡透皮贴在肝癌患者术后镇痛中的临床应用效果。方法将109例行肝切除术的肝癌患者分为观察组(n=50)与对照组(n=59)。2组患者在术后2 d内均使用等剂量罗哌卡因+吗啡注射剂硬膜外镇痛泵进行镇痛治疗。之后观察组采用丁丙诺啡透皮贴进行术后镇痛;对照组采用按需镇痛,即在出现爆发性疼痛时予静脉滴注氟比洛芬酯注射剂镇痛。比较2组患者术后疼痛强度、爆发性疼痛次数、肝功能指标、住院时间及药物费用。结果观察组患者的术后爆发性疼痛次数较对照组少[(1.66±1.69)次vs(2.20±1.54)次,P<0.05)];术后肝功能指标、血常规指标、住院时间及药物费用情况在2组间比较,差异均无统计学意义(P均>0.05)。结论丁丙诺啡透皮贴联合硬膜外镇痛泵对肝癌患者术后镇痛效果良好,无明显肝功能损害,值得临床应用。
Abstract：AIM To evaluate the postoperative analgesic effect of a buprenorphine transdermal patch in patients with liver cancer following hepatectomy. METHODS One hundred and nine patients with liver cancer who underwent hepatectomy were randomly divided into 2 groups(observation group, n=50; control group, n=59). The observation group was treated with buprenorphine transdermal patch, while the control group received flurbiprofen axetil injection treatment according to the individualized pain when an breakthrough pain occurred. The postoperative pain intensity, freguency of breakthrough pain, liver function indicators, length of hospital stay and drug costs between 2 groups were compared. RESULTS The number of breakthrough pain in observation group was less than that in control group [(1.66±1.69) times vs(2.20±1.54) times,P<0.05], with no obvious liver damage. There were no statistical differences between 2 groups in liver function indicators, routine blood indexes, the length of hospital stay and drug costs, respectively(all P>0.05). CONCLUSION The buprenorphine transdermal patches have a better postoperative analgesia effect in patients with liver cancer who underwent hepatectomy, and the liver damage are mild, so it is worthy of clinic application.
