为加强医疗机构药事管理及促进合理用药，国家卫生健康委员会试点推行驻科药师工作模式。由于驻科药师服务规范与质量评价缺乏统一标准，制约了驻科药师工作模式的建立与完善。为此，由上海交通大学医学院附属第一人民医院牵头，联合多家医疗机构药学专家，围绕驻科药师的工作模式、服务规范及质量控制与评价三大核心问题制定本共识，旨在完善驻科药师服务体系，推动药学服务以患者为中心，提升患者获得感，引导药师深入临床发挥精准用药专业价值。
Abstract：As the national healthcare system reform deepens, the National Health Commission has piloted the unitbased clinical pharmacist program to enhance pharmaceutical management and promote rational drug use. However, the lack of standardized service protocols and quality assessment frameworks has hindered the integration of pharmaceutical services with clinical practice. To address this gap, the First People's Hospital of Shanghai Jiao Tong University School of Medicine, in collaboration with pharmacy experts from multiple institutions, developed a consensus focusing on working models, service standards, and quality metrics. This initiative aims to optimize the unit-based clinical pharmacist service system, enhance patients' satisfaction, and align pharmaceutical care closely with clinical and patient-centered needs, thereby maximizing pharmacists' role in ensuring medication safety and efficacy.
