银杏叶提取物片联合胞磷胆碱钠治疗脑白质疏松认知障碍的临床疗效及其可能机制Effects of Ginkgo biloba extract tablets combined with citicoline sodium in treatment of leukoaraiosis-cognitive impairment and its possible mechanism

摘要(Abstract):
目的评价银杏叶提取物片联合胞磷胆碱钠治疗脑白质疏松引起的认知障碍的临床效果及其可能机制。方法选取2015年1月至2017年1月经确诊为脑白质疏松合并认知障碍的82例患者,随机分为对照组和试验组,每组41人。对照组给予银杏叶提取物片40 mg,tid;试验组予银杏叶提取物片40 mg+胞磷胆碱钠胶囊0.2 g,tid。2组患者连续服用12周。分别在服用前及服用1、3、6个月后通过随访对患者的认知功能、日常生活能力,以及社会功能活动能力进行评估,在服用前和用药6个月后进行脑源性神经生长因子(BDNF)和神经生长因子NGF的测定。结果 2组患者认知能力Mo CA评分治疗前为(18.52±3.72)和(18.23±3.24)分(P>0.05),治疗后,试验组(28.74±3.14)分,显著高于对照组(23.87±2.51)分(P<0.05);日常生活能力ADL评分在治疗前对照组(38.23±5.10)分,相比试验组(39.21±4.87)分,差异无统计学意义(P>0.05),治疗后试验组(48.21±3.85)分显著高于对照组(44.87±4.22)分(P<0.05);社会活动能力FAQ评分在治疗前对照组(1.38±1.12)分,相比试验组(1.40±1.18)分,差异无统计学意义(P>0.05),而治疗后试验组(6.82±1.14)分,显著高于对照组(3.25±2.00)分(P<0.05)。试验组患者BDNF和NGF的含量也高于对照组和用药前(P<0.05)。结论银杏叶提取物联合胞磷胆碱钠可以明显改善脑白质认知障碍的认知功能、生活能力,对于病情的进展有着重要的延缓作用,其作用可能与BDNF和NGF的表达提高有关。
